Role: Senior Director leading enterprise population health initiatives and community outreach to improve care access and patient outcomes in underserved communities. Responsibilities: develop and execute multi-year, disease-area-focused programs; translate population health strategies into actionable portfolios; lead flagship initiatives from conception through scaling; build and manage cross-sector partnerships; and communicate strategy and progress externally. Requirements: Bachelor's degree; 10+ years in healthcare, public health, or related fields with experience in population health, community-based programs, and stakeholder engagement; proven success in managing complex enterprise initiatives and building partnerships.
