Dedicated Case Coordinator Great Lakes

Role: The Inflammation and Immunology Dedicated Case Coordinator (DCC) manages patient access and reimbursement support for prescribed PDPA and VFM products. Objectives: Own the entire patient journey from enrollment through post-fill follow-up, ensuring seamless benefit investigation, prior authorization, and financial navigation. Responsibilities: Serve as primary contact for patients and healthcare providers, conduct benefit verifications, coordinate appointments and assessments, manage copay and interim care programs, triage prescriptions, and maintain detailed case documentation. Collaborate cross-functionally with HUB partners, clinicians, payers, and vendors to resolve access barriers and ensure compliance. Qualifications: B.S./B.A. with 8+ years in pharmaceutical or related customer-facing roles, including at least 6 months experience with Polaris CRM System and reimbursement processes. Skills: Strong communication, relationship-building, knowledge of specialty medication access pathways, and proficiency in case management tools. High-Value: Focus on specialty immunology/inflammation products, patient access, reimbursement, and payor interactions. Work setup: Remote-based, with 10-20% travel, primarily in Eastern or Central time zones.
