Position Summary
As Care Coordinator, you coordinate patients, caregivers/families, and healthcare providers when Praxis therapy is prescribed—owning the end-to-end experience from onboarding through adherence, monitoring for access/adherence barriers, and managing a complex caseload of patients with rare/refractory CNS conditions.
Primary Responsibilities
- Make day-to-day case decisions with escalation for critical inflection points.
- Independently manage difficult caregiver conversations and non-routine access situations; share learnings with the team.
- Own the end-to-end journey (enrollment → adherence) for caregivers and providers.
- Serve as primary point of contact; communicate clearly, timely, and empathetically.
- Communicate benefit investigation results; educate on financial assistance.
- Coordinate benefits investigations, prior authorizations, and financial assistance with stakeholders.
- Build relationships with specialty pharmacy and internal/external partners; adapt communication to audiences.
- Partner with specialty pharmacies on initiation, shipment, and ongoing adherence.
- Identify/resolve access and adherence barriers; intervene early based on engagement/adherence signals.
- Document interactions accurately in Salesforce and other PSP systems.
- Identify case patterns and recommend SOP/training/system enhancements.
- Report adverse drug events as required.
What You Bring
- Bachelor’s degree + 5+ years case management in/supporting pharmaceutical patient support programs.
- Knowledge of pharmacy benefits, prior auth/appeals, specialty pharmacy operations, and patient assistance programs.
- Experience with caregiver/family support and collaborating with field teams and specialty pharmacies.
- Proficiency with Salesforce and PSP platforms; experience with free goods/PAP business rules.
- Preferred: nurse/MA/social worker/senior reimbursement specialist experience; CNS therapy experience.
Compensation & Benefits (selected)
- Annualized base salary: $86,000–$106,000.
- 99% premium paid for medical/dental/vision; company-paid life/AD&D/disability; 401(k) match up to 6%; quarterly bonus; PTO and paid holidays.
