Care Coordinator, PSP HUB

Role: Patient Care Coordinator supporting Praxis's CNS therapies, managing patient cases from onboarding to adherence, and acting as the primary point of contact for caregivers, patients, and healthcare providers. Responsibilities: independently handle complex case management, facilitate therapy access and adherence, coordinate benefit investigations, prior authorizations, and financial assistance, and build relationships with internal and external stakeholders including specialty pharmacies. Requirements: Bachelor's degree, at least 5 years of case management experience within or supporting pharmaceutical patient support programs, strong knowledge of pharmacy benefit processes, and experience working with CNS therapies or rare CNS conditions preferred. High-Value: CNS (brain disorders), rare/refractory conditions, patient support programs, cross-functional collaboration with specialty pharmacies, clinical and reimbursement teams, remote work with flexibility for travel. Operates with ownership, clarity, urgency, empathy, and collaboration across the full patient journey.
